Tdap & Meningococcal Vaccine Requirements

All seventh graders are required to have a Tdap (tetanus/diphtheria/pertussis) immunization, within the five-year period before entering seventh grade. A Td (tetanus/diphtheria) immunization during the same time period will also meet the requirement, but parents should speak with their child’s health care provider about whether a Tdap booster should also be administered. If your child has received a Tdap or Td immunization within the last five years, a copy of the immunization record should be sent to the school nurse. Otherwise, please share the information on this immunization form with your child’s physician or Health Department clinic. The completed form should be returned to the nurse at Creekside or Crossroads middle schools.
For more information, please call Creekside Middle School at 513-829-4433; or Crossroads Middle School at 513-829-4504.
Meningococcal: One (1) dose of meningococcal (serogroups A, C, W, and Y) vaccine is required for all pupils in grades 7- 11.Meningococcal vaccine given before 10 years of age should not be counted as part of the adolescent MenACWY series. If a child received a dose of either MPSV4 or MenACWY before age 10 years, they should receive a dose of MenACWY at 11 or 12 years and a booster dose at age 16. A second (2nd) dose of meningococcal (serogroups A, C, W, and Y) vaccine is required prior to entry into the 12th grade. The second (2nd) dose must be administered on or after the 16th birthday with at least eight (8) weeks between the first (1st) and second (2nd) dose. If the first (1st) dose of meningococcal (serogroups A, C, W, and Y) vaccine was administered after the 16th birthday, a second (2nd) dose is not required. If a pupil is in 12th grade but is 15 years of age or younger, only one (1) dose is required. The pupil should then receive a second (2nd) dose when they turn 16 years.
