Does your 7th Grader Need a Tdap?

The Ohio Department of Health added an immunization requirement for all students entering the seventh grade, beginning with the 2010-11 school year. All seventh graders are required to have a Tdap (tetanus/diphtheria/pertussis) immunization, within the five-year period before entering seventh grade. A Td (tetanus/diphtheria) immunization during the same time period will also meet the requirement, but parents should speak with their child’s health care provider about whether a Tdap booster should also be administered. If your child has received a Tdap or Td immunization within the last five years, a copy of the immunization record should be sent to the school nurse. Otherwise, please share the information on this immunization form with your child’s physician or Health Department clinic. The completed form should be returned to the nurse at the Middle School.
