Attention Athletes and Parents/Guardians: Please read the following announcement about Final Forms, the electronic system for submitting the required athletic forms:

The FCSD Athletic Department uses a paperless form system called Final Forms. All forms, including physical forms, must be completed and uploaded online. Please visit 

https://fairfield-oh.finalforms.com/  to complete the required forms. The system will guide you through the process. Once all forms are submitted, you will only be required to update and sign forms once per school year online. The online update will be a few minutes at most! A physical will still be required each year.  A copy of the physical form is in the file list below. See "ATHLETICS - Preparticipation Physical Evaluation Form."
