Student Registration Center

The Student Registration Center is accepting new enrollments and address changes electronically through FinalForms. Please see the additional links on this page for instructions and a list of documents needed to complete your enrollment. Once your forms are completed online, you can upload the Items Required to Complete Enrollment to your child's folder in FinalForms or email them to registration@fairfieldcityschools.com. Parents are not required to present documents in person to our Registration Office.

Your child's enrollment will not be complete until all necessary documents have been received and reviewed.  You will receive a confirmation email with your child's school building assignment once the enrollment with Fairfield City Schools is complete.

The Student Registration Center follows the district calendar and all weather-related closing and delays.

  • How do I sign up?

    1. Go to FinalForms.
    2. Locate the parent icon and click NEW ACCOUNT.
    3. Type your YOUR NAME, DATE OF BIRTH, and EMAIL. Next, click REGISTER.

    NOTE: You will receive an email within 2 minutes prompting you to confirm and complete your registration. If you do not receive an email, then check your spam folder. If you still can not locate the FinalForms email, then email support@finalforms.com informing our team of the issue.

    1.  Check your email for an ACCOUNT CONFIRMATION EMAIL from the FinalForms Mailman. Once received and opened, click CONFIRM YOUR ACCOUNT in the email text.
    2. Create your new FinalForms password. Next, click CONFIRM ACCOUNT.
    3. Click ADD STUDENT for your first child (See Step 2).
  • What information will I need to complete the online forms?

    Birth certificate, previous school contact information, basic health history, IEP/ETR/MFE details (if applicable).

    How do I register my first student?

    IMPORTANT: If you need to create a parent account, please follow the steps on “Step 1 – Create a FinalForms Parent Account”

    1.         Go to FinalForms
    1.         Click LOGIN under the Parent Icon.
    1.         Locate and click the ADD STUDENT button.
    1.         Type in the LEGAL NAME (exactly as printed on the birth certificate) and other required information. Then, click CREATE STUDENT.
    1.         Complete each form and sign your full name (i.e. ‘Jonathan Smith’) in the parent signature field on each page. After signing each, click SUBMIT FORM and move on to the next form.
    1.         When all forms are complete, you will see a ‘Forms Finished’ message.

     

    How do I register additional students?

    Click MY STUDENTS. Then, repeat steps number 3 through number 6 for each additional student.

    How do I update information?

    Login at any time and click UPDATE FORMS to update information for any student.

  • The following items are required for each student you are registering.

    *Registration cannot be completed if all items are not provided*

    • All registration forms completed and signed in FinalForms. This includes the New Student Registration Process form.  Please be sure all enrollment forms have turned green.
    • Student’s Birth Certificate
    • Parent/Guardian Photo ID
      • Driver’s license/state ID
      • Passport/green card
    • Proof of Residency
      • Do you own/rent your home? You will need ONE of the following:
        • Current utility bill* - must show matching service and mailing address
        • Mortgage statement/settlement statement
        • Property tax bill
        • Current and signed lease/rental agreement
      • Living with a friend/family member? You will need ALL of the following:
        • Residency Affidavit - Must be signed by a homeowner/leaseholder in the presence of a Fairfield City School District official or Notary Public
        • Homeowner/leaseholder photo ID
        • Proof of residency as listed above
        • Current piece of business mail* or Ohio driver's license in parent/guardian name with address of residence

    *Current mail is less than 3 months old. Such as a paycheck stub, bank statement, phone bill, insurance, or government document. Cannot be hand written, junk mail, or a package.

    • Custody Documents (must be court-stamped)
      • Divorce decree and Shared Parenting Order
      • Court-issued custody order
      • Grandparent Power of Attorney
    • Immunization Records (Must be provided within 14 days of start date; student may be excluded from school until records are provided)
  • Grades K*-5:

    Parents should contact the student's assigned building to confirm the start date and get classroom assignment/schedule.  In most cases, students will be able to begin attending two (2) school days after the enrollment process is completed and you have received a confirmation email.

    *PLEASE NOTE:  Fairfield's kindergarten is a half-day program.  Please be sure to know AM or PM assignment before your child's first day of attendance.

    Grades 6-8:

    In most cases, students will be able to begin attending two to three (2-3) school days after the enrollment process is completed and you have received a confirmation email.  A school counselor will contact you regarding your child's schedule and confirm his/her start date.  If you have any questions or concerns regarding your student's first day, please reach out to your assigned building's guidance office:

    Creekside:  513-829-4433 ext 6509

    Crossroads:  513-829-4504 ext 7511 (student last name A-L) or ext 7512 (student last name M-Z)

    Grade 9:

    Call 513-829-8300 to make an appointment with your child's school counselor for scheduling.  The school counselor will provide you with information regarding your child's start date.

    Grades 10-12 (Senior High Building):

    To schedule an appointment, please call the Counseling Department Secretary at 513-942-2999.  Your child will not be able to attend school until his/her schedule has been created.  After you schedule a meeting with the school counselor, they will provide you with information regarding your child's start date. 

    *PLEASE NOTE:   Bus transportation is available only for 10th graders.

  • Fairfield City Schools has partnered with FinalForms, an online forms and data management service, to simplify the back-to-school process for our families, while reducing our paper usage. FinalForms allows you to complete and sign enrollment, back-to-school, and athletic/club participation forms for your students.

    FinalForms saves data from season to season and year to year, meaning you will never need to enter the same information twice! FinalForms also pre-populates information wherever possible for each of your students, also saving you time.

    You may review your data at any time throughout the school year to verify it is current.  Please be sure to log in and update your information if:

    • You move (updated proof of residency must be uploaded to your child’s folder in FinalForms or emailed to registration@fairfieldcityschools.com)
    • Your phone number has changed
    • You would like to update your emergency contacts
    • There is a change in custody or marital status

    You will be required to sign your forms once per year and after any update or as additional forms are added. For grades 6-12, some forms will require a student's signature. Instructions for parents and students to access their FinalForms account are below.

    PARENTS:

    If you are a Fairfield parent who already has a Fairfield FinalForms account: Please sign in and complete back-to-school forms for the 2026-2027 school year.

    Please note:

    • All forms for the upcoming school year will be available for your review, updates (if applicable), and signature on July 1, 2026.
    • The Babysitter/Daycare Transportation Form will be electronic and available on FinalForms for the 2026-2027 school year. Alternate addresses only need to be provided if you are requesting school bus transportation to or from an address other than your residence. If your child rides to or from school on a daycare bus/van, you do not need to provide an alternate address.
    • Additional forms may be added to your student’s FinalForms account throughout the summer, or even into the school year. Please be sure to check your FinalForms account occasionally to be sure all forms have been signed, and all contact information is up to date.

    If you are currently a Fairfield parent who does not have a FinalForms account, please email the Student Registration Center at registration@fairfieldcityschools.com with the Parent/Guardian name and the name(s) of all your attending students. Once the information is provided to the Registration office, you will receive an email from FinalForms asking you to confirm your parent account. You will then be able to complete your student's Emergency Medical Authorization and back-to-school forms.

    STUDENTS:

    Newly enrolled and re-enrolled students will not be assigned an Office 365 account until the day after their enrollment with Fairfield has been completed.*  All currently enrolled students should already have an Office 365 account. Instructions for accessing that account can be found in the Student/Parent Tech HUB.

    *Students in grades 6-12 enrolling for the 2026-2027 school year will be assigned an Office 365 account by July 26, 2026. Athletes will then be able to access FinalForms and complete all required forms in time for August 1, 2026 practices.

    For any student who has never accessed their Fairfield email account through Office 365: You must log in to your Office 365 account. Once you have logged on to Office 365 for the first time, it will take approximately 15-30 minutes for your email account to be activated. Instructions for accessing your Fairfield Office 365 account can be found in the Student/Parent Tech HUB.

    To access your FinalForms account:  After your Fairfield email account has been activated, go to FinalForms, click LogIn under Student, and then click "Never logged in?" Provide your Fairfield student email address and click Send Secure Link. A confirmation email from FinalForms will be sent to your student email account. If you receive an error message, please email the Student Registration Center at registration@fairfieldcityschools.com for assistance.

    Back-to-school forms can only be signed by a student if the parent/guardian has completed their required forms.

    Only students in grades 6-12 are required to sign forms via FinalForms.  Students in grade 6 will receive in-school instruction on use of their laptop, access to their Office365 account, and logging into school resources including Final Forms.

    Students who have already logged into FinalForms via their Fairfield student email can disregard this message.

    Are you a Fairfield City or Township resident whose child will be 5 years old by August 14, 2026?

    KINDERGARTEN REGISTRATION FOR THE 2026/2027 SCHOOL YEAR IS OPEN!

    Early registration helps us make important decisions about the number of high-quality kindergarten teachers we will need in our six elementary schools. Please complete your registration as soon as possible!


    All registration forms must be completed in FinalForms.  Directions for creating a parent account and completing your registration forms can be found in the links below.  If you already have a current parent account with FinalForms, you can add your incoming kindergarten student to your account and complete the online enrollment forms.

    Once you have completed your online forms, you will need to upload the following items to FinalForms: 

    • Student’s birth certificate
    • Parent/Guardian photo ID
    • Applicable court documents, such as:
      • Divorce decree and Shared Parenting order
      • Court-issued custody order    
      • Grandparent Power of Attorney
    • Proof of residency - Please see the provided list for more detailed information regarding what will be accepted as proof of residency.

    You can also email these documents to registration@fairfieldcityschools.com.  Parents are not required to present documents in person to our Registration Office. 

    Please don't wait until the last minute to register for the 2026/2027 school year.  Share this information with your family and friends!

  • Children must be age 3 and potty-trained to enroll in the Fairfield City Schools preschool program as a typically developing student.

    All registration forms must be completed in FinalForms.  Directions for creating a parent account and completing your registration forms can be found in the links below.  If you already have a current parent account with FinalForms, you can add your incoming preschool student to your account and complete the online enrollment forms.

    In addition to your online enrollment forms, you will need to upload the following items to FinalForms: 

    • Student’s birth certificate
    • Parent/Guardian photo ID
    • Applicable court documents, such as:
      • Divorce decree and Shared Parenting order
      • Court-issued custody order    
      • Grandparent Power of Attorney
    • Proof of residency - Please see the provided list for more detailed information regarding what will be accepted as proof of residency.

    You can also email these documents to registration@fairfieldcityschools.com.  Parents are not required to present documents in person to our Registration Office. 

    Appointments are available if you need assistance with the Registration process or require an interpreter.  Click here to return to Student Registration Home Page in order to set up an appointment.

    Once your Preschool enrollment has been completed, you can expect the following:

    1. Children who are not on an IEP will be placed on a wait list for placement.  Parent will be contacted and provided with information about school/session/teacher once a placement has been made.

    2. Children who are on an IEP will be enrolled with an anticipated start date 2 days from the date enrollment is completed.  A representative from the preschool department will communicate the school/session/teacher information and start date with the parent.

    3. If you have requested an evaluation because of a suspected disability, you will be contacted regarding meeting dates and times for the evaluation.  Once it is determined that your child qualifies for services, you will be given more information about school/session/teacher and start date.

    4. If your child has been evaluated and determined not to have a disability, you can choose to have your child placed on the wait list or have your enrollment removed from Final Forms.

  • Intradistrict transfer requests are for students who live in one area of Fairfield or Fairfield Township and wish to attend a school different from their assigned home school.  Intradistrict transfer requests are limited and only granted if space is available at the requested school.

    Requests for the 2026-2027 school year (School starting in August 2026) will be accepted online only, beginning April 6, 2026.  The link to the online request form will be available starting at 8:00 AM on April 6, 2026.  Paper request forms will NOT be processed.  Notification of approval or denial will be provided no later than the week prior to the start of the school year.  Please note that requests submitted after May 1st may not be reviewed until August.

  • Open Enrollment is closed to New Applicants for the 2026-2027 School Year

    Relocation Applications:

    Resident students who are moving out of the Fairfield City School District and would like to finish 2026-2027 school year in Fairfield City Schools should fill out a Relocation Open Enrollment application and update their address by logging in to their FinalForms account. The Relocation Open Enrollment application is available for printing or may be picked up at the Administration Building (4641 Bach Lane).  Completed applications should be submitted to the school's principal.  Please provide a copy of the current proof of residency along with parent/guardian’s current photo ID by uploading them to your student’s FinalForms folder or emailing them to registration@fairfieldcityschools.com

    A student eligible to be a Relocation Open Enrollment applicant is a student who: 

    1. Has been living in Fairfield
    2. Is currently enrolled in Fairfield City Schools as a resident student
    3. Desires to stay in the district even though residence is changing to another school district
    4. Has uninterrupted enrollment in Fairfield City Schools during the current year and previous semester (cannot withdraw, enroll in another district and then apply to come back under relocation open enrollment)
    5. Must have paid all school fees (current and past) in full, qualified for a fee waiver or have an active payment plan in place

    If a relocation application is approved, the student becomes an open enrollee from the date of approval forward.  Parents must submit a RENEWAL open enrollment application for the 2027-2028 school year during the Open Enrollment application window for the student to be considered for open enrollment in Fairfield City Schools next school year.
