FinalForms

Fairfield City Schools has partnered with FinalForms, an online forms and data management service, to simplify the back-to-school process for our families, while reducing our paper usage. FinalForms allows you to complete and sign enrollment, back-to-school, and athletic/club participation forms for your students.

FinalForms saves data from season to season and year to year, meaning you will never need to enter the same information twice! FinalForms also pre-populates information wherever possible for each of your students, also saving you time.

You may review your data at any time throughout the school year to verify it is current. You will be required to sign your forms once per year and after any update or as additional forms are added. For grades 6-12, some forms will require a student's signature. Instructions for parents and students to access their Final Forms account are below.

Athletic Forms

Attention Athletes and Parents/Guardians

The FCSD Athletic Department uses a paperless form system called Final Forms. All forms, including physical forms, must be completed and uploaded online. The system will guide you through the process. Once all forms are submitted, you will only be required to update and sign forms once per school year online. The online update will be a few minutes at most! A physical will still be required each year.

If you are a Fairfield parent who already has a Fairfield Final Forms account: Please sign in and complete back-to-school forms for the 2025-2026 school year.

 

Please note:

  • All forms for the upcoming school year will be available for your review, updates (if applicable), and signature on July 1, 2025.
  • The Babysitter/Daycare Transportation Form will be electronic and available on Final Forms for the 2025-2026 school year. Alternate addresses only need to be provided if you are requesting school bus transportation to or from an address other than your residence. If your child rides to or from school on a daycare bus/van, you do not need to provide an alternate address.
  • Additional forms may be added to your student’s Final Forms account throughout the summer, or even into the school year. Please be sure to check your Final Forms account occasionally to be sure all forms have been signed, and all contact information is up to date.

 

If you are currently a Fairfield parent who does not have a Final Forms account, please email the Student Registration Center at registration@fairfieldcityschools.com with the Parent/Guardian name and the name(s) of all your attending students. Once the information is provided to the Registration office, you will receive an email from Final Forms asking you to confirm your parent account. You will then be able to complete your student's Emergency Medical Authorization and back-to-school forms.

Students
Newly enrolled and re-enrolled students will not be assigned an Office 365 account until the day after their enrollment with Fairfield has been completed.* All currently enrolled students should already have an Office 365 account.

*Students in grades 6-12 enrolling for the 2026-2027 school year will be assigned an Office 365 account by July 26, 2026. Athletes will then be able to access Final Forms and complete all required forms in time for August 1, 2026 practices.

For any student who has never accessed their Fairfield email account through Office 365: You must log in to your Office 365 account. Once you have logged on to Office 365 for the first time, it will take approximately 15-30 minutes for your email account to be activated.

Account Access Instructions

 

To access your Final Forms account:

After your Fairfield email account has been activated, go to Final Forms, click LogIn under Student, and then click "Never logged in?" Provide your Fairfield student email address and click Send Secure Link. A confirmation email from Final Forms will be sent to your student email account. If you receive an error message, please email the Student Registration Center for assistance.

 

Back-to-school forms can only be signed by a student if the parent/guardian has completed their required forms.

Only students in grades 6-12 are required to sign forms via Final Forms.

Students who have already logged into Final Forms via their Fairfield student email can disregard this message.

Parent Square

The Fairfield City School District uses ParentSquare for school communication, primarily with email, text, and app notifications. ParentSquare automatically generates an account for each parent, using their preferred email address and phone number. We encourage parents to access their accounts so they can download the mobile app and update their preferences on when and how they are notified.

With ParentSquare, you can:

  • Receive all district, school and classroom communication via email, text, or app
  • View the school and classroom calendar and RSVP for events
  • Choose to receive information as it comes or all at once with a daily digest in the early evening
  • Communicate in your preferred language
  • Securely receive report cards, IEPs and other important student documents
  • You can use ParentSquare on any device. You can download the free mobile app for iOS or Android or use the desktop version at the ParentSquare website.

Rapid Notification System

We are using ParentSquare as the rapid notification service to communicate district, school, and classroom announcements. New student and parent accounts will be created in August when the new student schedules are uploaded.

Parent Square FAQs

  • You don’t have to take any action to receive emails, texts, or phone calls. Our student information system syncs contact information directly with ParentSquare. If you want to utilize the app, simply download it from Google Play or the App Store and follow the prompts.

  • Yes. The first time that you log into the app or web portal, all of your students should automatically be listed in the upper left-hand corner. If you don’t see them, please contact us at info@fairfieldcityschools.com

  • You can choose to receive messages via email, text, voice, and/or app notifications. While you can opt out of any of these methods of communication, please note that the district has the discretion to override opt-outs for messages and alerts deemed urgent or important, including health and safety situations, school closures, and attendance.

  • You can customize your notification settings by going to your homepage and clicking on your name in the top right corner and selecting "My Account" from the drop-down menu. On your account page, you find your notification settings in the top right corner and click "Change This" to customize your notifications. If you’re receiving too many notifications, try changing to the "digest" setting, meaning you’ll only receive one condensed message in the evening.

    How to Change Your Notification Settings

  • Please check your spam to see if any ParentSquare messages ended up there and mark them as "Not Spam." Also add “donotreply@parentsquare.com” to your email contacts so your server recognizes our messages.

  • You can customize your language settings by going to "My Account" and clicking "Change This" under language settings. From there, you can choose any language that you prefer from the drop-down menu, and you’ll start to receive content in that language.

    How to Change Your Language Settings

  • ParentSquare allows the district, schools, and teachers to send messages in a variety of ways:

    • One method is a post that would only go to your school’s “feed” in the ParentSquare app or web portal. Your school or teacher also could choose to send that post to parents/guardians in the form of a text or email.

    • Staff can choose to send a private message to one or more users. For example, a school nurse might text you to ask a question about administering your student’s medication.

    • The third method is a “smart alert” sent at the district and school level — things like the district newsletter, messages from the superintendent, and messages about back-to-school nights. They can be sent via text, email, or phone call.

    • The final category is an “urgent smart alert.” These messages will be sent via text, email, and phone call and include inclement weather notifications and health and safety issues.
