Immunization Verification Info for FHS Seniors

Dear Parent/Guardian of Junior Students,
The Ohio Department of Health has an immunization requirement for all students entering the twelfth grade in 2017. All twelfth grade students will now be required to have a 2nd meningococcal immunization prior to entering the twelfth grade.
Many students may have already received the required immunization. Please check with your child’s health care provider. If your child has received the immunization, have the physician sign and then fax or mail the documentation to the Fairfield High School Nurse as soon as possible. All official vaccination records signed by the health care provider will also be accepted.
If your child has NOT received the required immunization, please make an appointment for your child to do so as soon as possible. This information must be sent to Nina Rose, Fairfield High School Nurse, no later than April 1, 2017.
Any students with NO documentation of receiving the 2nd immunization on file will be excluded from entering the twelfth grade.
Please share this information with your child’s physician or health clinic. If you have no current physician or health care provider, you may call the Primary Health Solutions at 513-454-1111 or contact them at www.myprimaryhealthsolutions.org and schedule an appointment to receive the required immunization. All completed forms should be returned to Nina Rose, Fairfield High School Nurse.
