FAIRFIELD CITY SCHOOL DISTRICT
Requirements for Substitute Teacher/ESL Tutor/Home Instructor/Summer School Teacher
- Ohio Teaching Certificate/License or Ohio Substitute Teaching License– A photocopy is acceptable. The Substitute Teaching License application may be downloaded from the Ohio Department of Education website www.ode.state.oh.us or we have them in our District Office. You will need a check or money order made out to Treasurer, State of Ohio, for either a five-year ($125) or one year ($25) substitute license. NOTE: If you choose to apply for a one-year license, and you plan to renew it for next year, another criminal background check for BCII and FBI will be required. While the five-year license is a greater expense up front, it may be more cost effective in the long run.
- Transcripts - Your transcripts (Official, not photocopied) showing a minimum four (4) year degree awarded.
- Criminal Background Record Checks (BCII and FBI) – Please call our District Office at 829-6300 Monday through Friday, between the hours of 8:00 a.m. and 4:00 p.m. to make an appointment for fingerprinting. We are no longer accepting copies of background checks.
COST: $22 BCII / $24 FBI / $46 for Both (we require both)

(NO CHECKS)
On the fingerprint appointment day, bring your current/valid Ohio Drivers License or photo ID. NOTE: All criminal background/fingerprints are directed to the Ohio Dept. of Education, 25 South Front St, Columbus, OH 43215, and to Fairfield City Schools, 211 Donald Dr, Fairfield, OH 45014.
- If you are applying to be a substitute nurse or R.N., you will need to bring in your current nursing license. You will also be required to have background checks as above.
6. Payroll Information - You will complete any necessary payroll information at the mandatory New Substitute Orientation Meeting. If you sub in any position before the meeting, you must contact Sue Hollingsworth (513) 858-7114 or hollingsworth_s@fairfield-city.k12.oh.us in our payroll department to schedule an appointment to complete your necessary payroll information.
