All students in grades 7 through 12 participating in interscholastic sports sponsored by Fairfield City Schools must have a physical examination.
The pre-participation physical form must be completely filled out and signed. Forms not completely filled out cannot be accepted. The pre-participation physical form is valid for one school year. The form must be turned in to the coach before or on the official start date of that particular sport.
