**Kindergarten Registration Information for 2012-2013**
It's time to round up your kindergartners! Kindergarten registration for the 2012-2013 school year is now under way at the district Welcome Center, and will be held in each of the five elementary schools in April. Parents will also be able to register during two special Saturdays. The dates and locations are:
March 1 - District Welcome Center, 211 Donald Drive from 8 a.m.-2:30 p.m. Parents/guardians are encouraged to register their incoming kindergartners by June 30.
April 11 - South Elementary, from 5-8 p.m.
April 12 - North Elementary, from 5-8 p.m.
April 18 - Central Elementary, 5-8 p.m.
April 24 - East Elementary, 5-8 p.m.
April 26 - West Elementary, 5-8 p.m.
Saturday enrollment dates are at the Welcome Center from 8 a.m.-noon and are April 21 and May 5.
The following information is required prior to registration being finalized:
• Completed registration packet
• Birth Certificate
• Parent/Guardian's driver's license, passport or state identification
• Proof of residency, which includes current rental/lease agreement, mortgage statement, settlement statement, deed, or property tax statement. Note: If a parent/legal guardian's name does not appear on the proof of residency, the person whose name appears on the document (this person must also be an established district resident) must come with the parent/legal guardian to prove residency. If this applies, any one of the following is required for additional proof of residency:
1. Parent's new Ohio driver's license with current address
2. Parent's U.S. Mail forward confirmation with new address
3. Parent's current business mail received to new address
• Custody papers (court-stamped divorce decree with shared parenting order, if shared parenting is filed. Any modifications or custody orders relevant to child.)
