ATTENDANCE INFORMATION

Fairfield Schools are using an automated phone calling system to advise parents if their child is absent from school. The call will be made for all absent students, unless a note or email has been received from the parent. Parents who call the school to advise their child will be absent will still receive the automated call, as a reminder to send a note regarding the absence when the child returns to school. Does your seventh-grader need a Tdap immunization?

The Ohio Department of Health has added a new immunization requirement for all students entering the seventh grade, beginning with the 2010-11 school year. All seventh-graders are required to have a Tdap (tetanus/diphtheria/pertussis) immunization, within the five-year period before entering seventh grade. A Td (tetanus/diphtheria) immunization during the same time period will also meet the requirement, but parents should speak with their child’s health care provider about whether a Tdap booster should also be administered.

If your child has received a Tdap or Td immunization within the last five years, a copy of the immunization record should be sent to the nurse at the Middle School.

Otherwise, please share the information on this immunization form with your child’s physician or Health Department clinic. The completed form should be returned to the nurse at the Middle School.
