| Ohio Teaching Certificate/License or Ohio Substitute Teaching License– A photocopy is acceptable. The Substitute Teaching License application may be downloaded from the Ohio Department of Education website www.ode.state.oh.us or we have them in our District Office. You will need a check or money order made out to Treasurer, State of Ohio, for either a five-year ($125) or one year ($25) substitute license. NOTE: An initial license requires a BCII and FBI background check. A renewal license requires an FBI background check every five years. |

| Transcripts - Your transcripts (Official, not photocopied) showing a minimum four (4) year degree awarded. |

| Criminal Background Record Checks (BCII and FBI) – Please call our District Office at 829-6300 Monday through Friday, between the hours of 8:00 a.m. and 4:00 p.m. to make an appointment for fingerprinting. We will accept certified copies of background checks if they are dated within 365 days. |

| If you are applying to be a substitute nurse, you will need to bring in proof of your current RN license. You will also be required to have background checks as above. |
